How To Apply
Hillholm
Residents are selected according to Ministry of Health and Long Term Care guidelines and Hillholm House program criteria.
Applicants must:
- be over 16 years of age
- have severe and persistent mental health problems
- be willing to participate in a rehabilitational and recovery-focused program
- be able to live in a congregate living situation
Inquiries may be made by anyone, but an application must be filled out by a professional mental health worker in conjunction with the applicant. A copy of the application form can be downloaded by clicking here (PDF format-Adobe Acrobat is required). The completed form should be sent to:

Beverley House
In keeping with the program requirements of the Ministry of Health and Long-Term Care, applications for Beverley House come through referrals from partner agencies that provide case management services to Beverley residents. At this time, our partner agencies are the Hostel Outreach Program of Community Resource Connections of Toronto (416.482.4103 or hop-mgr@crct.org) and the COMPASS program of Toronto East General Hospital (416-461-2205).
Applicants must:
- be women and over 16 years of age with a recent history of homelessness
- have severe and persistent mental health problems
- be willing to participate in a rehabilitational and recovery-focused program
- be able to live in a congregate living situation
